Take the Improve YOU Vitality Health Review

The Vitality Health Review (VHR) is a confidential online survey that takes about 15 minutes to complete. It asks simple questions about your health habits and helps you see if you may be at risk for preventable illnesses that you can address with simple lifestyle changes.

Want to save money on your 2015 medical premium? The VHR is one of three steps you’ll need to complete by November 7, 2014, to earn an employee wellness credit applied to your 2015 medical premium—$10 if you get paid biweekly or $5 if you get paid weekly.

To Access the Improve YOU Portal, You Must Take the VHR

To take the VHR, from your work computer go to SAP Employee Self Service (FMC Technologies network logon required) > Your Total Rewards > Improve YOU.

Not on your work computer or you’re a Completion Services employee? No problem! You can still access Vitality. Choose the option that works best for you:

  • Go to www.yourtotalrewards.com/fmcti, log on (you will need to create a username and password the first time you log on) and select Improve YOU;
  • Go to www.PowerofVitality.com You’ll need to create a username and password. Your spouse or domestic partner can also access Vitality by logging on to www.PowerofVitality.com. The first time your spouse or domestic partner logs on, he or she will need to:
    • Enter his or her first name, last name, date of birth, and the last four digits of the employee’s Social Security number.
    • Create a username and password; or
  • Download the Vitality Today mobile app from the iTunes App Store or Google Play Store. If you’ve logged on to www.PowerofVitality.com before, you must use the same username and password. Otherwise, you will need to create a username and password.

 

Earn the Employee Wellness Credit to Save on Your 2015 Medical Premium

Get healthier and save money! Your 2015 medical plan cost per pay period will be determined by your participation in the Improve YOU wellness program. You can receive an employee wellness credit* of $260 applied to your 2015 medical premium.

All you have to do is complete the VHR, have a Vitality Check biometric screening and reach Silver Vitality Status by November 7, 2014.

If you complete these steps, you’ll receive an employee wellness credit applied to your 2015 medical premium—$10 if you get paid biweekly or $5 if you get paid weekly.

*Your spouse or domestic partner can also take the VHR and Vitality Check (through their doctor only) to learn more about their health, but they are not eligible for the employee wellness credit.

Step 1: Take the Vitality Health Review

The VHR is a confidential online health questionnaire.* You will need to answer a few health-related questions to get a personalized summary of your current health and risk factors. It’s not only easy, it’s quick—the VHR only takes about 15 minutes to complete.

To take the VHR, from your work computer go to SAP Employee Self Service (FMC Technologies network logon required) > Your Total Rewards > Improve YOU.

Not on your work computer or you’re a Completion Services employee? No problem! You can still access Vitality even if you’re not on the FMC Technologies network. Choose the option that works best for you:

  1. Go to www.yourtotalrewards.com/fmcti, log on (you will need to create a username and password the first time you log on) and select Improve YOU; or
  2. Go to www.PowerofVitality.com. You’ll need to create a username and password. Your spouse or domestic partner can also access Vitality by logging on to www.PowerofVitality.com. The first time your spouse or domestic partner logs on, he or she will need to:
    • Enter his or her first name, last name, date of birth, and the last four digits of the employee’s Social Security number.
    • Create a username and password; or
  3. Download the Vitality Today mobile app from the iTunes App Store or Google Play Store. If you’ve logged on to www.PowerofVitality.com before, you must use the same username and password. Otherwise, you will need to create a username and password.

Once you complete the VHR, you’ll be able to start earning points by completing healthy activities and goals. Your points can help raise your Vitality Status® and can be redeemed for rewards.

Step 2: Give your health a Vitality Check!

The Vitality Check* is a biometric screening similar to the one you were previously required to complete to earn an employee wellness credit. The Vitality Check determines your body mass index (BMI), blood pressure, cholesterol and other metabolic markers, such as glucose and triglyceride levels. You must get your Vitality Check and have your data submitted to Vitality by November 7, 2014.

For the most accurate results from your Vitality Check, remember to fast (no eating any food or drink other than water) 9 to 12 hours prior to your screening. It's a good idea to drink plenty of water, and you can still take all medications as prescribed by your physician.

We're offering many ways to get your screening, so you're sure to find one that fits your busy lifestyle.

Option 1. On-site at select work locations

We know how valuable your time is, so we're bringing the Vitality Check biometric screening to you! On-site screenings are free and will be held at select work locations from September 8 through October 17.

Sign up today for a screening. You'll need to reserve a spot soon. It's easy!

  • Go to www.my.blueprintforwellness.com.
  • Under New Participant, enter your registration key: FMCOnsite. This key provides you with access to schedule a screening appointment at an on-site event at an FMC Technologies location.
  • Enter your SAP employee ID number and your date of birth. To find your SAP employee ID number:
    • Go online to SAP Employee Self Service.
    • Select Resource Finder under Employee Information.
    • Enter "name or e-mail" and select Search. Your SAP employee ID number will be the 8-digit ID listed under Employee NO.
    • Note: If you’re a Completion Services employee, you will need to contact your local HR Representative for your SAP employee ID number.
  • Click Submit.
  • Follow the on-screen directions to confirm your demographic information.
  • Create a username and password.
  • Select Schedule Your Screening.
  • To schedule your on-site screening appointment, select Schedule Your Onsite Event Based Screening.

Can't attend an on-site screening? Don't worry. There are other ways to get your screening to help you save on your 2015 medical premium.

Option 2. Quest Diagnostics Patient Service Centers

You can get a free Vitality Check at a Quest Diagnostics Patient Service Center (PSC). Go to www.my.blueprintforwellness.com to find a location near you and to schedule an appointment.

To schedule an appointment:

  • Enter your registration key: FMCPSC. This key provides you with access to schedule a screening appointment at a PSC close to your home or place of work.
  • Enter your SAP employee ID number and your date of birth.
  • Click Submit.
  • Enter your information and follow the prompts.
  • Select Schedule Your Screening > Attend a Quest Patient Service Center at your convenience.
  • Follow the on-screen steps to place an order to visit a PSC.
  • You may schedule an appointment at a PSC near you or select to attend as a walk-in. Wait time for walk-ins can average 20 minutes.
  • Bring the confirmation page with you to your appointment.

Option 3. Doctor's office*

Both you and your spouse or domestic partner can get your Vitality Check biometric screening at your doctor's office. To qualify for the employee wellness credit, you'll need to complete a Vitality Check form (your spouse or domestic partner can earn Vitality Points). Go to Your Total Rewards:

  • To access the Your Total Rewards website, just go to SAP Employee Self Service (FMC Technologies network logon required) > Your Total Rewards > Improve YOU.
    • Or if you’re a Completion Services employee or you're not on the FMC Technologies network, go to www.yourtotalrewards.com/fmcti and log on. You will need to create a username and password the first time you log on. Click Improve YOU.
  • Select Get Healthy > Vitality Check Form to print the form and take it to your doctor.
  • Your doctor will need to complete, sign and e-mail the Vitality Check form to This email address is being protected from spambots. You need JavaScript enabled to view it. or fax it to +1 877.224.7110. Be sure to submit the form by November 7, 2014.

Note: If you already had your biometric screening or physical at the doctor's office in 2014, then you don't have to do it again. Just send the form to your doctor and ask your doctor to compete and sign the form, and then submit it to Vitality by e-mail at This email address is being protected from spambots. You need JavaScript enabled to view it. , or by fax at +1 877.224.7110.

*Your physician visit will be covered as directed under the health plan design, applicable to any deductibles, co-pays or co-insurance.

Step 3. Reach Silver Vitality Status

You'll earn points for completing the VHR and Vitality Check. You also earn points when you complete wellness activities. These points are called Vitality Points. The more Vitality Points you earn, the higher your Vitality Status. You must reach Silver Vitality Status by November 7, 2014, to be eligible for the employee wellness credit for 2015.

To reach Silver Vitality Status, you will need to earn 2,500 points if you do not have a spouse or domestic partner in the program. If your spouse or domestic partner is eligible to earn points, the two of you will need to earn a combined total of 3,500 points to reach Silver Vitality Status.

Your Spouse or Domestic Partner Is Invited Too!

Spouses and domestic partners can access the Improve YOU portal and complete their own VHRHRA so they can start earning their own Vitality Points. All points earned by a spouse or domestic partner will count toward the employee’s Vitality Status. However, the spouse or domestic partner will not be eligible for a wellness credit.

Does FMC Technologies Ever See My Results or Personal Health Information?

No. FMC Technologies never has access to any of the personally identifiable survey information you provide in your VHR or your Vitality Check.

  • All individual answers and personal information are 100% confidential.
  • HIPAA privacy guidelines are strictly followed.
  • FMC Technologies will only have access to group (not individual) results. This is for reporting and tracking purposes only.
